Before you make a claim
Check your policy to make sure your condition meets the definition of disability and that you are eligible for the benefit.
Also note that to receive benefits, the number of days you're disabled must be longer than the waiting period indicated in your policy.

Documentation
Fill out the Disability claim form and ask your doctor to complete the Physician's statement.
Gather any other supporting documents required. Then send everything through our secure site or by mail (address listed on the form).
Good to know
You're responsible for any fees your doctor charges to complete the medical statement.

Analysis
Once we receive the required documents, we review your claim. Then we contact you to let you know if your claim is approved. We also establish the payment method, if necessary.

Payment
If your claim is approved, you receive your benefits after the waiting period.
What's the waiting period?
To receive your monthly benefit, you must wait a certain amount of time after the date you became disabled.
