How to get your group insurance claims report

Learn how to download your group insurance claims report from our secure site.

Instructions

  1. Log in to the secure site. On the home page, select Claims history, then the Claims reports for income tax purposes link.

    Note

    If you don’t have this option, contact your plan administrator.

  2. On the Claims reports for income tax purposes page, choose the appropriate tax year from the dropdown list.

  3. Select the Request this report button.

  4. You’ll see your report in the Consult my reports section.

    Note

    If you request a new report later in the year, it will replace the previous one. Reports are saved on the site for 7 years.
